Who are we?
Reach Out Camden is an alliance of organisations supporting people in Camden.
The Alliance provides a collaborative, community-based approach to wellbeing services and acts as a Single Point of Access for multiple community wellbeing partner organisations. Reach Out Camden is a new collaborative approach to community-based wellbeing services. This collaborative approach is known as an Alliance.
It is really important to us that you feel safe providing us with your information, and we will look after it carefully.
Reach Out Camden acts as a Single Point of Access for three support providers and an organisation leading on service user involvement. It is important to note that this Privacy Notice is supplementary to the individual Privacy Policies/notices of each contributing alliance.

1. The information we collect
We only collect information that is necessary to provide our services, and we will look after it carefully. On the referral form, we collect general personal details about you and your needs as well as equality, diversity and inclusion data. Equality, diversity, inclusion, and health data are ‘special category data’, which you have the right to choose not to share. Sharing this information is voluntary and is used only to help us better understand your needs and improve our ability to offer appropriate, tailored support.
When we start working with you, we may take notes relating to our work with you and we may also collect information from other services or individuals in your support network.
2. Lawful basis for processing personal data
We process this information primarily on the basis of your consent (you giving us information, such as within a referral form) and legitimate interests to ensure appropriate support and service coordination.
3. What we do with it
We use this information to provide our services across Reach Out Camden. We may also use it to evaluate and improve our services. We are required to provide some anonymised data to our commissioner, Camden Council. We only keep information as long as is reasonable and required by social care guidelines. You have a right to access your information at any point and request for us to delete or amend it.
Data supplied to Reach Out Camden is held within the UK.
4. Does anyone else see my information?
In order to provide you with our services, we may need to share some of your information with our partner organisations or relevant members of your support network. The only other time we may share your personal information is if it is required by law, or to protect against harmful or illegal activity.
We would not sell or rent your information to third parties. We do not share your information with third parties for marketing purposes.
5. How long do we keep your information for and how it is kept?
Information we hold are all on restricted access and limited to only those who are entitled to carry out their duties.
