Making a referral
About the process
Completing our referral form is the first step towards accessing support through Reach Out Camden.
It should take around 10-15 minutes to complete and will ask you a few questions about you, your mental health and your life more generally. We understand that talking about these things isn’t always easy and could stir up some challenging emotions. We encourage you to take your time and seek some additional support if you need it. After submitting this form, a Reach Out Camden Navigator will get in touch with you to arrange an initial ‘Get To Know You’ meeting.

What you can expect
We know that each person contacting Reach Out Camden will bring with them a unique story. We work with everyone in a person-centred way throughout their time with us and your preferences, hopes and goals will
define your experience with us.
This means everyone’s journey will be slightly different, but here is how you can expect your time with Reach Out Camden to begin.

Getting to know you

You, or someone you know such as a friend or GP, makes a referral to Reach Out Camden. This can be done online, over the phone, by post, or in-person at one of our drop in events. Find out more about making a referral

A Reach Out Camden Navigator will contact you to arrange an initial ‘Get To Know You’ meeting.

When you meet, you’ll talk with your Navigator about what made you get in touch, what kind of support you’re looking for, and begin to explore together options that may be of interest to you.
If it’s really clear after this chat which of our support offers you’d like to try, then great! Once you’ve decided on a direction, we’ll help to make sure everything’s in place for you to get started as soon as possible.
If it’s not so clear, that’s also completely fine. You’ll have time to talk it through with your navigator and make a decision

Do I have to complete the form online?
No, there are several other ways you can make a referral to Reach Out Camden:

Do I have to fill in everything on the form (is every field mandatory)?
No, only some details are required – these are clearly indicated on the referral form. Other questions are optional at the referral stage, but they may be important to discuss with your navigator.
Do you provide Mental Health Advocacy?
We do not provide mental health advocacy. Our focus is on community connections and support.

Why are questions about ethnicity, gender, or sexual orientation asked?
We believe everybody should have access to our services regardless of age, gender, disability, race, sex, sexual orientation, marital status, socio-economic status, religion or belief. These questions are not asked to exclude anyone from our services, rather they ensure we are providing a diversity of services for the whole community. Our aim is to work alongside people and adapt our offers so there is something for everyone.
We are a non-judgemental team who welcome all people just as they are. We understand these questions can feel quite personal, so if you would feel more comfortable filling out this information in person with someone, just leave the section blank for now
Is the information I share kept confidential?
Once you make a referral to Reach Out Camden, we take great care to make sure your information is looked after and kept safe. You can read our full privacy statement here (link), but some key points covered are:
- The information we collect from you is used to deliver and evaluate our services, and we only collect information that is necessary for us to do this.
- You have a right to access information we keep about you at any time, and ask us to delete or amend it.
- The only other time we would share information externally to Reach Out Camden would be if it is required by law, or to protect against harmful or illegal activity.

Can I complete the form on behalf of someone else?
Absolutely, people are often referred by GPs or other people in their support network. Please just check with the person you’re referring that they are happy for you to contact us on their behalf and that they consent to you sharing their information with us.
